The Social Worker is responsible for providing emotional, psychological, and social support to patients and their families. The role focuses on assessing patient needs, providing counseling services, and connecting patients with appropriate community resources to improve their well-being and recovery process.
Key Responsibilities:
- Assess patients’ social, emotional, and psychological needs.
- Provide counseling and emotional support to patients and their families.
- Assist patients in coping with illness, hospitalization, and treatment processes.
- Work closely with doctors, nurses, and healthcare teams to support patient care.
- Develop individualized care plans for patients requiring social support.
- Assist patients in accessing community resources and social services.
- Provide guidance on financial assistance programs if applicable.
- Support discharge planning and post-hospital care arrangements.
- Maintain accurate documentation of patient cases and interventions.
- Conduct patient interviews and family consultations when necessary.
- Participate in multidisciplinary team meetings and case discussions.
- Advocate for patients’ rights and well-being.
- Provide crisis intervention support during emergencies.
- Educate patients and families about available support services.
- Maintain confidentiality of patient information.
- Monitor patient progress and adjust support plans when required.
- Ensure compliance with hospital policies and healthcare regulations.
- Participate in community outreach and awareness programs.
- Prepare reports and documentation related to patient support services.
Requirements:
- Bachelor’s degree in Social Work or related field.
- Valid SCFHS professional classification required.
- Experience in healthcare or hospital environment preferred.
- Strong empathy, communication, and counseling skills.
- Ability to work effectively with diverse patient populations.
- Good organizational and documentation skills.
