We are looking for Registered Nurses to join our clinical team and deliver high-quality home care services to adult and pediatric patients. This role is ideal for compassionate, patient-focused nurses who are committed to safety, quality, and evidence-based clinical practice.
🩺 Role Purpose
- Provide safe, effective, and patient-centered nursing care in a home care setting
- Conduct clinical assessments, develop and implement care plans, and evaluate outcomes
- Work closely with the Clinical Care Manager and multidisciplinary team
- Supervise and support Assistant Nurses and Healthcare Assistants when required
🔑 Key Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and communicate updates to physicians
- Ensure patient safety, confidentiality, and adherence to clinical policies
- Support discharge planning and continuity of care
- Act as Case Manager when assigned
- Participate in quality improvement initiatives and staff training
🎓 Qualifications & Experience
- Bachelor’s Degree in Nursing (minimum 3 years program)
- Valid DHA or DOH Registered Nurse license
- Valid BLS certification
- Minimum 2 years post-qualification clinical experience
