Qualified Healthcare Professionals – Home Care
Job Summary
We are seeking qualified, compassionate, and experienced Healthcare Professionals to provide safe, high-quality, patient-centered care in clients’ homes. The successful candidate will deliver clinical and personal care based on individual care plans, monitor patients’ health conditions, support treatment plans, and work closely with doctors, nurses, families, and other healthcare professionals.
Key Responsibilities
- Provide professional healthcare services to patients in their homes according to approved care plans.
- Assess and monitor patients’ physical, emotional, and clinical needs.
- Administer medications and treatments as prescribed and within professional scope of practice.
- Monitor vital signs and recognize changes in patients’ health conditions.
- Provide wound care, injections, catheter care, IV-related care, and other clinical procedures where appropriately qualified.
- Assist patients with activities of daily living, mobility, nutrition, hygiene, and personal care when required.
- Support patients with chronic illnesses, disabilities, post-operative recovery, and age-related healthcare needs.
- Provide appropriate health education and guidance to patients and their families.
- Maintain accurate, timely, and confidential patient care records.
- Communicate changes in patients’ conditions promptly to the supervising clinician or relevant medical professional.
- Coordinate with doctors, nurses, therapists, caregivers, and family members to ensure continuity of care.
- Follow infection prevention, medication safety, safeguarding, and occupational health procedures.
- Respond appropriately to emergencies and arrange escalation or medical assistance when necessary.
- Respect patients’ privacy, dignity, cultural preferences, and personal choices.
- Ensure safe handling and appropriate use of medical equipment and healthcare supplies.
- Participate in care reviews, clinical meetings, training, audits, and quality-improvement activities.
- Maintain professional boundaries, confidentiality, ethical standards, and all applicable healthcare regulations.
Qualifications
- Relevant recognized healthcare qualification, such as:
- Registered Nurse / Staff Nurse
- Licensed Practical/Vocational Nurse
- Physiotherapist
- Occupational Therapist
- Respiratory Therapist
- Other appropriately licensed healthcare professionals, depending on the service provided.
- Valid professional registration/license in the relevant jurisdiction.
- Relevant clinical or home healthcare experience is preferred.
- Current BLS/CPR certification is an advantage.
- Additional qualifications relevant to the patient's condition or specialty are desirable.
