Job Title: Home Care Coordinator
Job Summary
A Home Care Coordinator is responsible for planning, organizing, and managing in-home healthcare services for patients. The role ensures that patients receive appropriate medical and personal care at home by coordinating between patients, families, caregivers, and healthcare professionals.
Key Responsibilities
- Assess patient needs and develop individualized home care plans
- Coordinate scheduling of home visits by nurses, caregivers, and therapists
- Assign and manage home care staff based on patient requirements
- Communicate with doctors and healthcare providers regarding patient care plans
- Monitor patient progress and update care plans as needed
- Ensure quality and continuity of care in home settings
- Maintain accurate patient records, reports, and documentation
- Educate patients and families about care procedures and treatment plans
- Ensure compliance with healthcare regulations and safety standards
- Handle emergency situations and escalate to medical professionals when necessary
- Manage supplies, equipment, and logistics for home care services
Required Qualifications
- Bachelor’s degree in Nursing, Healthcare Administration, or related field (preferred)
- Experience in nursing, caregiving, or healthcare coordination
